Department of Health Services (DHS), Division of Licensing Services at 602-364-2690 has List of Providers including Nursing Home Providers throughout the state and a searchable List of Facilities organized by facility type.
Other Helpful Information:

• The Arizona Health Care Cost Containment System (AHCCCS) at 1-800-962-6690 provides low or no cost medical coverage programs to eligible Arizonans. For example, one of the AHCCCS programs, Arizona Long Term Care System (ALTCS) at 1-800-654-8713 provides coverage to eligible Arizona senior citizens. Applications are available online or at your local ALTCS office.
  
  
• Benefits Check-Up® allows senior citizens to Pre-Screen for eligibility to ALTCS and other programs. Arizona Self Help also allows individuals to Pre-Screen for eligibility to ALTCS and other programs.
