MEDICAL CARE FOR DISABLED

The U.S. Social Security Administration (SSA) at 1-800-772-1213 provides benefits to the elderly, blind and people with disabilities. You may apply by phone, mail or in person at any Social Security office. Social Security benefit applications are available on-line. There is no cost to apply.

• The Social Security Benefit Eligibility Screening Tool (BEST) allows individuals to Pre-Screen for eligibility to any of the programs Social Security administers.
  
  
• The Department of Economic Security (DES), Rehabilitation Services Administration at (602) 542-3332 assists people with disabilities by providing medical care information and referrals to support groups.
  
  
• The Centers for Independent Living (CIL) provides services to disabled people of all ages and with all types of disabilities.
  
  
• The Arizona Health Care Cost Containment System (AHCCCS) at 1-800-962-6690 provides low or no cost medical coverage programs to eligible Arizonans. Applications are available on-line.
